
I. Requirements of Admissions
A. Age Requirements:
Students applying for Pre Pre-Kinder to
Kindergarten must be 3 years old (PPK),
4 years old (PK), and 5 years old
(Kinder) by September 30. We will be
able to accept only 30 applications for
Pre-Pre Kinder, 50 applications for
Pre-Kinder, and 25 applications for
Kinder.
B. Documents Needed:
o
Completely filled-out and signed application
form.
o
Official transcript of credits from previous
school.
o
Letter of conduct from previous school.
o
Certificate of good financial standing from
previous school.
o
One 2x2 photo.
o
Student's Social Security Number (photocopy
of S.S. card).
o
Copy of birth certificate for grades
Pre-Pre-Kinder, Pre-Kinder, and Kinder.
o
Student Health Evaluation (form attached to
the application form).
o
Students applying for Pre-Pre-K must be
toilet trained and able to separate from
parent.
o
A non-refundable $30.00 application fee per
student for grades 1-12 and $70.00 for PK.
C.
All applicants must take an entrance exam
(Pre-Pre-Kinder - 12th).
Applicants for second through twelfth grade
take the Metropolitan Achievement Test (in
English). In addition, applicants for eighth
through twelfth grade take a math competency
exam. An interview with the students or
additional testing may be required at the
discretion of the admissions committee.
D.
No tests are administered unless all
admissions requirements have been completed.

II. Admissions Process
o
The documents already mentioned, the
application and the admissions test results
are considered by our Admissions Committee.
o
Parents will receive a written and/or phone
communication of the decision.
o
If the decision is affirmative, parents will
receive a letter stating due date for
payment of enrollment and other applicable
fees, the Student Health Evaluation (to be
filled out by a doctor), the official
vaccination record from the Commonwealth of
Puerto Rico (PVAC-3 form, provided by the
Health Department or pediatrician), the
Tuition & Fees Agreement, and the Drug and
Locker form.
o
Enrollment will be processed only with the
signed acceptance letter, tuition and fees
agreement, Student Health Evaluation, the
PVAC-3 form and the Drug and Locker form.
o
Information regarding book and uniform sales
will be mailed to parents. If you have not
received this information by May, please

o
If the admission of the student occurs after
school has begun, the orientation process
will be handled on an individual basis.
We reserve the right to make final decisions
about assignment of a student to a grade
section. Wesleyan Academy is open to
students of all or no religious backgrounds.
We make our opportunities available without
the consideration of any social or political
factors.
